What can be the cases of failure and share your experience?

Would you elaborate on the failure mechanism? There are too many ways failure could occur and bring specific will narrow the solution matrix.

Taj Singh BajwaWith any boiler, poor operational surveillance with respect to water quality is a common cause of failure. The water treatment programme, e.g. all volatile or phosphate, will dictate the surveillance and effort required.
The question is overly broad and expansive, so responses will tend to match the standard set.
